Seiler, Theo MD, PhD; Hafezi, Farhad MD Abstract: Purpose: Corneal collagen cross-linking by UVA/riboflavin (X-linking) represents a new method for the treatment of progressive keratoconus and currently is under clinical study. To avoid UVA irradiation damage to the corneal endothelium, the parameters for X-linking are set in a way that effective treatment occurs only in the first 300 [mu]m of the corneal stroma. Here, X-linking not only strengthens the biomechanical properties of the cornea but also induces keratocyte apoptosis. To date, the effectiveness of treatment could be monitored only indirectly by postoperative follow-up corneal topographies or using corneal confocal microscopy. Here we describe a corneal stromal demarcation line indicating the transition zone between cross-linked anterior corneal stroma and untreated posterior corneal stroma. The demarcation line is biomicroscopically detectable in slit-lamp examination as early as 2 weeks after treatment. Methods: X-linking was performed in 16 cases of progressive keratoconus, and corneas were examined biomicroscopically and by means of corneal topography and pachymetry before and after treatment. Results: In 14 of 16 cases, a thin stromal demarcation line was visible at a depth of approximately 300 [mu]m over the whole cornea after X-linking treatment. Conclusion: This newly observed demarcation line may result from differences in the refractive index and/or reflection properties of untreated versus X-linked corneal stroma and represents an effective tool to biomicroscopically easily monitor the depth of effective X-linking treatment in keratoconus. Nelle visite successive al trattamento ho visto con i miei occhi sulle immagini del microscopio quella linea di demarcazione e il dott. Seiler ha preso anche le foto. Seiler mi aveva detto infatti che quella sottilissima linea chiara che si vedeva nello stroma indicava proprio il cross-linking. Ciao. Simone. |

Le foto sono state prese perchè (credo) sono stato trattato durante il congresso internazionale sul cheratocono del dicembre 2005 che si è tenuto a Zurigo. All'epoca credo di essere stato uno dei pochi ad essere trattato con il cross-linking con lo scrubbing parziale dell'epitelio. Dal momento che la reazione al trattamento era stata corretta sono stato trattato con lo stesso metodo anche dall'altro occhio. L'esito dello scrubbing parziale è stato che il periodo post-operatorio è stato poco doloroso e tre giorni dopo il trattamento sono stato mandato via. Il recupero è stato veloce perchè i tratti dell'epitelio rimasti hanno funzionato come drivers per la rigenerazione del tessuto. Prima del'intervento c'era un oculista di New York a cui Seiler aveva mostrato la mia situazione, dal momento che il cross-linking viene effettuato ormai da 8 anni senza bisogno di essere replicato questo dottore aveva detto che in media ogni 10 anni le cellule della cornea si rinnovano completamente per cui si è sulla buona strada per pensare che il cross-linking non necessiti di essere replicato anche se è stato studiato nell'eventualità per essere replicato nel caso in cui vi sia la necessità.
